Midline incisions are the most frequently applied access in open abdominal surgery. It is simple ; it is quick to perform and close, provides adequate exposure to all four abdominal quadrants and usually blood sparing. A major problem after median laparotomy is the adequate technique of abdominal fascia closure. In prospective studies the incidence of incisional hernias varies from 9% to 20%. Wound infection, obesity and suture closure technique are addressed as major risk factors for the development of an incisonal hernia. There is no consensus regarding the strategy for closure of the abdominal fascia after midline and transverse incisions within the surgical community
