Endemic goiter in Croatia

In early fifties, goiter prevalence in continental Croatia varied from 46% in Zagreb to 80% in Slavonia and Lika. In the village Rude in 1949 goiter was found in 85% of schoolchildren (SC). The first law requiring the compulsory iodization of all salt for human and animal consumption, with 10 mg KI per kg of NaCl, was enacted in 1953. Ten years later a threefold reduction of goiter prevalence was recorded: in Osijek from 83% to 33%, in Požega from 75% to 33%, in Bjelovar from 51% to 35%, in Varaždin from 70% to 45%, in Sisak from 80% to 20% and in Karlovac from 62% to 21%. Renewed investigations on goiter incidence in early nineties, initiated by Kusić et. al., have demonstrated on pilot study in Zagreb area persistence of goiter incidence from 19% to 35%. Due to, Minister of Health appointed, in May 1992, governmental National Committee for Goiter Eradication and Iodine Prophilaxis, with task to establish the real prevalence of goiter in Croatia. This project has shown, over the 5 years, that mild iodine deficiency still persisted in Croatia, keeping goiter prevalence in our SC between 8% and 35%. Examinations of 2856 schoolchildren showed the following goiter rates: Zagreb 19%, Rude 35%, Osijek 28%, Ðakovo 22%, Vuka 30%, Delnice 25%, Rijeka 13%, Split 8%, Lovrec 14% and Supetar 11%. Urinary iodine excretion (UIE) measurements revealed that 48%-86% of SC had values above 5 ľg/dL, and 9%-34% of SC had UIE above 10 ľg/dL. Based on these results, on 9 October 1996., the Minister of Health proclaimed the new regulation on iodination of salt for human and animal consumption, requiring 25 mg KI per kg of NaCl.One year later, National Committee has tried to establish whether some improvement due to it's implementation could be observed. UIE was measured in of 589 SC from Zagreb, Rude, Osijek, Ðakovo, Rijeka, Pazin and Dubrovnik. For that purpose Rapid Urinary Iodide Test (MERCK KGaA, Darmstadt, Germany) was efficiently used. Medians of UIE ranged from 5.5 - 13,4 ľg/dL, and they were higher than before introduction of the new law (7,4 - 9 ľg/dL). These values are the best first signs of improved iodine intake in Croatian youth.
