Personalized Respiratory Medicine: The One-size-does-NOT-fit-all Approach to Interstitial Lung Disease Associated with Sjögren's Syndrome. - A Review.

Systemic autoimmune diseases such as Sjögren's syndrome (SS) are a heterogeneous group of immunologically mediated inflammatory disorders with pulmonary manifestations, the most prevalent and debilitating form of which is interstitial lung disease (ILD). The pathogenesis of pulmonary fibrosis development in ILD is incompletely understood but seems to involve a complex cascade of coagulation and inflammation within an autoimmune setting leading to a profibrotic environment and lung fibrogenesis. Currently available therapeutic options (i.e. anti-inflammatory and immunosuppressive drugs) have thus far been ineffective in reducing severity or progression of this chronic and lethal respiratory disorder. Lung transplantation remains the only curative option for ILD patients. Contemporary research is focussed on early diagnosis and monitoring of disease severity, as well as predicting progression and survival, utilizing peripheral blood biomarkers. Applying genomics and proteinomics to ILD research in SS patients is crucial to elucidating the role of genes in increasing susceptibility to development of pulmonary fibrosis with the goal of individualized, gene-targeted therapy. Personalized or "person-centered" medicine in respiratory disorders such as SS-associated ILD requires a broader vision - one that inspires a pro-active and preventive approach to the health and well-being of the whole person, benefitting both patient and health professional alike.
