Comparison of postoperative results after arthroscopic capsular release in primary frozen shoulder and posttraumatic shoulder stiffness

INTRODUCTION Arthroscopic capsulotomy and manipulation is a treatment of choice after unsucessfull nonoperative treatment of stiff shoulder. AIM To compare results of arthroscopic treatment of primary and secundary shoulder stiffness. METHODS We have operated 30 patients with primary shoulder stiffness and 13 patients with posttraumatic stiffness. In all cases arthroscopic capsulotomy and manipulation under OEA was performed. We have measured pasive ROM prior surgery, 24 hours after surgery (under continuous regional analgesia) , a month after and three months after surgery. RESULTS In primary group results are presented respectively ; preoperatively/24 hours after surgery/a month after surgery/3 months after surgery. Flexion ranged from 101±23(60-150) / 165±21(100-180) / 148±20(110-170) / 142±25(100-180). External rotation was ; 12±11(-10-30) / 64±20(20-90) / 33±7(20-45) / 38±23(0-80). Internal rotation ranged ; sacrum/Th12/L3/L2. In secondary group results are also presented respectively. Flexion ranged from 105±20(80-150) / 170±13(140-180) / 155±19(130-170) / 167±12(150-180). External rotation was ; 14±12(0-40) / 60±16(25-80) / 30±14(20-50) / 53±19(20-70). Internal rotation ranged ; L5/Th12/Th11/Th9. CONCLUSION First day after surgery ROM was improved in both groups, and than it decreases during first month when it reaches final values. Although there were no significance in ROM between two tested groups prior and after surgery, inside both groups there was significant improvement in all movements after surgery. The authors emphasize the importance of immediate postoperative rehabilitation, especially during the first month after surgery.
