Multimodal approach in the treatment of back pain and radicular pain – case report

Despite analgesic therapy and minimally invasive procedures of pain-treatment, the result is often unsatisfactory. The aim of this paper is to show that besides the physician ; also the patient has a very important role in a long-term reduction of pain. This is the case report of a patient with pain in the thoracic and lumbar spine, with the pain spreading bilaterally in the lower extremities. The patient's magnetic resonance image showed pathology of thoracic and upper lumbar spine. The patient was advised to take pregabalin, diclofenac and Zaldiar tablets as needed and reduce his weight (132 kg). We performed an epidural steroid injection, using the interlaminar approach at the level of LI / LII with 80 mg Depo Medrol and 10 ml of 0.25% levobupivacaine. Arriving to our clinic the patient had experienced pain for the last three months (VAS 10). After the epidural steroid injection, the condition was significantly improved, and the intensity of pain was 5 (VAS). At the follow-up visit the intensity of pain was decreased (0-5 by VAS). After five months the patient's analgesic therapy was cancelled with a pain intensity 0-3 (VAS) without pharmacotherapy. The patient successfully continued the weight-loss with physical exercise (swimming) and a changed lifestyle. After fifteen months the patient had lost 50 kg in weight (82 kg). Analgesic therapy with epidural steroid administration, lifestyle changes and weight loss has proven successful in reducing back and radicular pain with this patient.
