Factors related to patients' satisfaction with appearance of the gums

Objective: The shape of gingival tissues plays an important role in the esthetics of maxillary anterior teeth and the abnormalities in contour can significantly influence the level of patients’ satisfaction with their maxillary teeth appearance.The purpose of this study was to quantify the specific distal displacement of the gingival zeniths in the maxillary anterior dentition of young adults and to investigate their influence on the level of the patients’ satisfaction with the appearance of gums in different gender groups. Method: The distal gingival zenith displacements on all maxillary upper teeth were measured. All the patients rated their satisfaction with gum appearance on the visual-analogue scale (0 absolute dissatisfaction, 4 complete satisfaction). All the parameters have been analyzed with respect to gender. Result: The gingival zenith was distally displaced between 0.25 and 1.02 mm in 91%, 84% and 61% of the central incisors, lateral incisors and canines, respectively. Contralateral comparisons between the tooth groups revealed significantly larger distal displacement of the zenith in left canine (p<0.05). Ipsilateral comparisons revealed that the lateral displacement of the zenith was significantly larger in central than in lateral incisors (p<0.05). All the measurements were found to be significantly higher in men (p < 0.05). The majority of the participants were completely satisfied with the appearance of their gums (81% of men and 86% of women). In men, the gingival displacement of the upper lateral incisors influenced the variability of the patients’ satisfaction with gums appearance by 99% (p > 0.05). In women, the gingival displacement of the upper central incisors influenced the variability of the patients’ satisfaction with gums appearance by 74% (p > 0.05). Conclusion: Within the population tested, the results suggest that the use of gingival displacement measurements may serve as an esthetic guideline and should be considered in esthetic oral rehabilitation.
