Telemedicine in cardiac pacing

Cardiac pacemakers have sophisticated telemetry and diagnostics features. Event counters enable arrhythmias follow-up and adjustment of pacemaker function. Telemetry provides determination of remaining battery life and lead conductor status. Periodically, patients travel to pacemaker centre several hours, to complete the follow-up session in 10-40 minutes. Some malfunctions occur very fast and more frequent follow-up would obtain ultimate patient safety. Pacemaker programming and telemetry device with ECG recorder, connected to the ATM network of CARnet, have to be in a remote physicians office. Pacemaker centre workstation will be connected through CARner with tne remote device, thereby monitoring ECG and pacemaker programmer GUI. Accordingly, pacing expert could provide remote and complete patient follow-up from the pacemaker centre. In order to prove the feasibility of the procedure, we adapted available programmer and connected it to the Sun SparcStation server. We developed ECG interface having RS232 serial output, connected to the same server. Utilizing the WEB technology and server software developed in Java language, we were able to provide the client connected to our server with the real time ECG waveform display which enables remote patient monitoring. This is prerequisite for patient safety in any remote programming procedure. Network throughput is sufficient for diagnostic procedure in rhythmology. There is no hazard for the patient due to the network connection interruption.
