Neonatološki aspekti frenotomije u novorođenčadi s ankiloglosijom. Paediatr Croat 2013 ; 57:216-20.

Ankyloglossia or tongue-tie, is an congenital anomaly characterized by a short lingual frenulum, which can lead to short-and long-term restrictions of the tongue movements. In newborn period can causes feeding difficulty. Aim. The aim of this paper is to obtain ankyloglossia in clinical and functional presentation in otherwise healthy newborns, established the degree of breast-feeding difficulties in infants with ankyloglossia, and to evaluate the effectiveness of frenotomy. Methods. This retrospective analysis includes 2.872 of healthy newborns. Ankyloglossia is estimated clinically, identified the difficulties of breastfeeding, and the efficacy of frenotomy in newborns with severe ankyloglossia. Results. The frequency of ankyloglossia is 3.76%. The ratio of boys and girls was 3.1:1. Mild ankyloglossia was found in 27 (25%) infants, in 32 (29.63%) was moderate, and 49 (45.37%) had severe ankyloglossia. Transient difficulties with breastfeeding in infants with mild and moderate ankyloglossia were resolve through timely professional help during breastfeeding, without the need for frenotomy. In 37.04% of the total number of children with ankyloglossia, only in infants with severe ankyloglossia is concomitant difficulties breastfeeding (40 of 49 cases), underwent frenotomy. The most common difficulty is the nipple pain during breastfeeding (75%), attachment to the breast in 22.5%, and the nipple damage in 2.5%. Pain intensity before frenotmy was 7.38 + 1.53, and after frenotomy 1.75 + 1.11 (p <.0001). Conclusions. Proper assessment the degree of ankyloglossia and the need for frenotomy in newborns can resolve most difficulties with breastfeeding. Frenotomy in newborns is mildly, fast and safe procedure that can be performed in the first days of life, and long-term effectiveness should confirm by the randomized controlled trials.
