Sažetak
Objectives: The aim of the study was to assess survival rates and prognostic features affecting outcome of papillary thyroid carcinoma (PTC) patients (pts) treated and followed-up in one institution. A total of 1167 PTC pts (F:M ratio 4:1) operated between 1970–2009 with a mean follow-up 8.4 years (y) (range 0.5–41y) were included in the study. Median pts age at the time of diagnosis was 47y (6–83y). All pts underwent total/near total thyroidectomy and 95% pts underwent 131-I treatment. Median tumor size (MTS) was 12mm (1–100mm). Methods: Prognostic factors: age (<45y, >45y), gender, tumor size (<40mm, >40mm), presence of local and distant metastases at the time of diagnosis were evaluated with univariate (Kaplan- Meier Survival Curves, Log- rank test) and multivariate analysis (Cox Regression) (p < 0.05). Results: Local metastases were recorded in 252(27%) pts at initial presentation. Loco- regional recurrence developed in 59(5%) pts during follow-up. Distant metastases were recorded in 40(3.4%) pts at initial presentation (F:M ratio 1.3:1, MTS 33mm) and developed subsequently in 3 pts. Lung and bone metastases were the most frequent. 14 PTC pts died from disease (8 with distant meta and 6 with inoperable advanced loco- regional disease) (F:M ratio 0.6:1, MTS 45mm). Overall 5 and 10-year cancer-specific survival rate in PTC pts was 99.1% and 98.8% (80% with distant metastases). Univariate analysis demonstrated older age, male gender, larger tumor size, presence of local and distant metastases at the time of diagnosis as significant (p < 0.001) features affecting outcome. Multivariate analysis demonstrated older age (p = 0.05), larger tumor size (p = 0.01) and distant metastases (p < 0.001) as prognostic features affecting outcome. Conclusions: PTCs are malignant tumors with extremely low mortality. Distant metastases are rare and can be successfully treated. Older age, larger tumor size and presence of distant metastases are independent, negative prognostic factors that influence outcome of PTC patients.
