Background Lung cancer is the most common second primary cancer and TNF-α is a multifunctional cytokine involved in the pathogenesis of various inflammatory and malignant diseases We investigated whether TNF-α-238 and 308 polymorphisms were associated with the susceptibility and severity of second primary lung cancer (LC2) in Croatian patients. Methods Study included 104 patients, 98 with non-small cell lung cancer (NSCLC) and 6 with small cell lung cancer (SCLC). The control subjects included two groups: 201 patients with lung cancer as a first primary cancer (LC1) and 230 healthy blood donors (HC). The analysis of TNF-α gene polymorphisms was performed by PCR. Results The frequencies of the TNF-α-238 polymorphism GG genotype and the G allele were higher in the LC2 group relative to LC1 group (p=0.054 and p=0.057, respectively) and HC group (p=0.054 and p=0.057, respectively), but the differences did not reach significance. Patients with a second primary NSCLC more frequently had TNF-α-238 polymorphism GG genotypes and G alleles relative to patients with a first primary NSCLC (p = 0.060, p = 0.064, respectively). GG genotype was exclusive and no A allele was found in group LC2. There was no statistically significant difference in the distributions of the TNF-α-308 polymorphism genotype and alleles between LC2 and LC1 and HC groups. Conclusion TNF-α-238 polymorphism GG genotype and the G allele could have a promotional effect on the development of NSCLC at a group LC2.
