Complete lower denture on two implants: how to correct them – a case report

Background: Prosthetic rehabilitation of edentulous lower jaw is one of the most challenging and demanding procedures in dental medicine. In some patients not even ideally fabricated lower complete dentures satisfy their expectations as regards stability and retention, and therefore implant placement is chosen for them as the means of retention and stability. Aim of the study:Standard treatment of complete denture fabrication requires a high level of knowledge about dental prosthodontics. By correct planning of the implant placement site, surgical procedures and choice of appropriate means of retention, the patient is provided with a high level of function of the lower complete denture. Materials and methods:A female patient at the age of 82 with an upper metal-based complete denture and a lower complete denture retained by ball attachments on two Ankylos (3.5/10) implants visited our dental practice on a colleague's recommendation, who fabricated her dentures a year ago. The patient was extremely dissatisfied with their function. Insufficient retention and stabilization of the upper and lower complete denture was established by clinical examination. Too small denture extension was also noticed, as well as unnecessary strengthening of dentures with a metal framework. Bad functional impressions were diagnosed as the cause of problems, as well as incorrectly defined prosthetic plane and interarch relationships and inappropriate occlusal contacts. The choice of retention means on implants was also incorrect. The patient was provided with new complete dentures in the way that two functional impressions were taken, the prosthetic plane was correctly determined and the interarch relationships were determined by means of face bow and centric relation record. Due to lack of parallelism of implants higher than 10˚, Locator attachments were chosen as means of retention of the lower complete denture. When dentures were handed in, obligatory remounting of dentures in the articulator was performed, which ensures correct contacts in static and dynamic occlusion. Results:Three years after new dentures were fabricated, the patient has no problems, and she is satisfied with their function. Conclusions and clinical implications:Beside correct planning of the implant placement site and sugical procedures, for a high level of function of a lower complete denture it is necessary to choose appropriate means of retention, as well as to have a high level of knowledge about dental prosthodontics, which includes correct functional impression procedures and determination of interarch relationships.
