Peanut allergy is the most important cause of plant food allergy. The aims of the study were to determine the frequency of peanut allergy in birch allergic children, sensitization patterns in birch allergic children with and without symptoms after peanut consumption and molecular component(s) responsible for severe reactions. Patients and methods: 95 birch allergic children were divided in two groups according to symptoms upon peanut consumption (symptomatic group N=45, asymptomatic group N=50). 35 children had milder and 9 had severe symptoms. In all children prick test with peanut allergen, peanut specific IgE and peanut IgE components (Ara h1, 2, 3, 8) were analyzed. Oral provocation was done to confirm peanut allergy. Results: Peanut allergy was confirmed in 8/95 children (8%). Wheal diameter in peanut symptomatic children was significantly larger then in asymptomatic children (p<0, 001) and peanut IgE was higher (median 61 kIUA/L vs. 0, 3 kIUA/L, p<0, 001). In the symptomatic group there were significantly higher values of Ara h1 and Ara h2 (p<0, 001), while Ara h8 was not significantly different in two groups. Positive Ara h1 and Ara h2 are determinants of symptoms upon peanut consumption (OR 6, 9 and 5, 1), and Ara h1 is a determinant for severe symptoms (OR 1, 6).Conclusion: Peanut allergy was more frequent in birch allergic children than in average population. In birch allergic and peanut allergic children there is larger peanut prick diameter, higher values of peanut specific IgE and Ara h1 and Ara h2 IgE than in birch allergic, peanut non-allergic children. Symptoms upon peanut consumption are to be expected in Ara h1 and Ara h2 positive patients, while Ara h1 is a single determinant of severe symptoms.
