Chest radiography in Emergency Department: who needs a normal finding

Introduction: Chest radiography (CR) is the most widely used radiologic test in Emergency Departments (EDs). There are no clear guidelines on prescribing CR in emergency setting. Multiple factors influence the decision to perform CR. We analysed 11-day period of ED visits with focus on radiologic work-up retrospectively. Methods: The date from electronic database on patient medical hsitory, physical exam and radiologic work-up as well as outcome (admittion, discharge and recommendations) were collected. A total of 613 patients were analyzed (20- 31/12/2012). Descriptive statistical analysis was made using Microsoft Office Excell 2010. Results: Out of 613 patients, 38.9% underwent urgent CR with 53.6% normal chest X-rays. Out of total findings, 32.6 led to or supported the main ED diagnosis. Most common pathologies were cardiomegaly (59.1%9, pulmonary congestion (34.9%), and pleural effusion (28.7%). CR was more often performed in older patients (51.2% for patients older than 65 years). Conclusion: Despite low cost and availability, CR is time and personel consuming procedure and poses substentional exposure to radiation. The large proportion of normal findings suggests clear recommendations or guidelines are needed.
