Comparison of diagnostic sensitivity and specificity of Troponin-T, CK, CK-MB in patients with acute myocardial infarction
The aim of this study was to investigate the value of the new marker Troponin T for use in patients with chest pain and suspected myocardial infarction. Thirty-three patients admitted to the emergency department with chast pain initiated within the preceding 12 hours of admission with suspect myocardial infarction were included in our study. Serial blood samples were taken 12, 36, 84, 120, and 168 hours after onset of pain. Troponin T was measured with an enzyme-linked immunosorbent assay (the sensitivity limit at 0- 10m/L. Catalytic activities of CK was measured by the method proposed by IFCC and CK-MB by immunoinhibition. The results showed the sensitivity of Troponin-T for detecting AMI increased significantly according to the number of hours elapsed after onset of chest pain from 17% for patients presenting within 4 hours to 71% for patients presenting in the interval og greater than 8 hours from onset chest pain (p<0.001). Specificity evaluated in the three time intervals ranged from 83% to 93%. A concordant result between CK-MB measurement and Troponin-T assay was observed in 135 (82%) tests. In 12/165 tests a positive troponin T and a negative CK-MB assay was observed, in 8/12 (66%) cases this results was true positive for Troponin T assay and in 4/12 (33%) cases false negative. The results suggested a cardiosensitivity as time dependent marker Troponin T was sensitive as CK and CK-MB in the early phase od MI with a high sensitivity of 100% even after 96 hours. Myosin showed high sensitivity (80%) in the late phase of MI. Troponin T and myosin are both high cardiospecific versus CK.
