Clinical evaluation of the CELL-DYNâ 1700CS blood counter

This paper presents evaluation of a CD 1700CS (Abbott Diagnostics), an instrument performing complete cellular blood count and leukocyte three-part differential from one measurement. Within-run and between-run precision showed satisfactory results for all parameters. Between-day precision was satisfactory too for most parameters, except for leukocyte and platelet count in the abnormal low range, and for mid-cell percentage in the whole range. Accuracy of the measurement tested by comparison MEK-8118K (Nihon Kohden) and CD 1700CS results showed good correlation coefficients (r>0.965) for nearly all parameters (WBC, RBC, HGB, HCT, MCV, MCH, PLT), except for MCHC (r=0.389). Carryover was found to be negligible, i. e. less than 1% for all parameters tested. Testing of linearity produced satisfactory coefficients of correlation for all the three parameters tested (HGB r=0.999, WBC r=0.998 and PLT r=0.998) and no deviation from linearity was detected throughout the whole range of concentrations tested (HGB =25-205 g/L, WBC =3.3-32.7 x 10 exp9 /L, PLT =89-890 x10 exp9 /L). The three-part differential blood count measured by the CD 1700CS was compared to the reference manual method of microscopic leukocyte differentiation, whereby satisfactory correlation coefficients were obtained for granulocytes (r=0.924) and lymphocytes (r=0.939). The proportion of mid-size cells, in contrast, showed a poor correlation (r=0.435) with the cumulative total of immature cells, monocytes, eosinophils and basophils calculated from the manual differential. The study of the diagnostic performance of the differential leukocyte count showed a good global sensitivity (92.1%) and specificity (88.6%), with a low number of false negative results (5.6%). Evaluation of the suspect population flags produced by the analyzer showed these flags to be a useful indication of the need of microscopic differentiation.
