DDrug induced reactions to aspirin and nonsteroidal anti-inflammatory drugs
Aspirin (ASA) and nonsteroidal anti- inflammatory drugs (NSAIDs) hypersensitivity is presented by urticaria, angioedema, asthma, rhinosinusitis, anaphylaxis, and with a variety of late cutaneous and organ-specific reactions. The pathomechanisms include the inhibition of cycloxygenase-1 that leads to shunting of arachidonic acid metabolism towards the 5- lipoxygenase pathway, which results in increased levels of cysteinyl leukotrienes in cross-reactive patients, and formation of specific immunoglobulin E antibodies to ASA or a single NSAID. There also are type IV hypersensitivity reactions. The diagnosis includes understanding of the pathomechanism and is necessary for prevention and management.
