Positive and expiratory pressure vs. recruitment maneuvers during anesthesia for laparoscopy.
Patients undergoing laparoscopic surgery almost always have some degree of respiratory impairment due to development of respiratory atelectasis and the decrease in functional residual capacity (FRC). Application of Positive End Expiratory Pressure (PEEP) during anesthesia can attenuate these effects, but application of PEEP is often linked with hemodynamic changes that some patients find hard to tolerate. Furthermore, surgeons sometimes also require a Trendelenburg position for the patient which than further exaggerates the effects of pneumoperitoneum on respiratory function and hemodynamic. The study was designed to compare the continuous use of PEEP during laparoscopic surgery with a double recruitment maneuver at the end of surgery.
