Ultrasonography of the optic nerve sheath in brain death
Recent studies showed correlation betweeen increased intracranial presurre(ICP) and ultrasonography of the optic nerve sheath diamter (ONSD). The ONSD limit range of 5.0-5.86 mm correlates with raised ICP(> 20cm H2O). The aim of this study was to assess the usefulness of ONSD ultrasonography in patients with brain death.Twentyfive patients (16 males) with confirmed brain death were evaluated by ONUS. Demographic data were obtained. ONSD was measured with a 12-MHz linear ultrasound probe adjusted ti give a suitable angle for displaying the entry of the optic nerve into the globe, at the depht at 3mm behind the globe. By rotating the probe clockwise, four measurements were made for each optic nerve, twice in transverse and twice in sagittal plane. The mean ONSD values were compared between the patients and 42 healthy controls. The mean ONSD in brain death patients ( 0.72+/-0.06cm) was statistically significantly higher as compared with controls (0.55+/-0.05 ; p<0.01). Measurements of ONSD can be a useful diagnostic noninvasive tool in distinguishing brain death individuals from healthy controls.
