Croatian External Quality Assessment for Coagulation Parameters: five years experience

In order to promote the WHO,s suggestion concerning the organisation of National External Quality Control (EQC) we have established a programme for the EQC of blood coagulation parameters in Croatia. EQC surveys have been organized continuously since 1994. four times per year, for determination of prothrombine time (PT) and fibrinogen. Since 1997. activated parial thromboplastine time (aPTT) determination was included in programme. Thromboplastins used in laboratories were of different sources and various sensitivity, some even without declared ISI value. The PT results were usually expressed as percentage. There has been a trend towards the use of high sensitive thromboplastins. In the last survey 74% laboratories used thromboplastins with ISI less than 1,2. Interlaboratories CV for PT results expressed as percentage is now 8,3 and for INR CV is 6,7 Interlaboratories CV for fibrinogen, determinated mostly by Clauss method, and aPTT expressed as ratio (test/normal) in the last survey was 9 and 10 respectively. Continuous updating of knowledge throught National External Quality Control and educational communication between participants and organiser as well as introduction of automated coagulometers seem to have contributed to standardisation and improvement of coagulation tests results.
