Sažetak
β-lactam agents such as penicillins, cephalosporins, monobactams and carbapenems are antibiotics of choice to treat a variety of incfections. The introduction into therapy was rapidly followed by the reports of resistance. Production of ESBLs (extended-spectrum β-lactamases) is the major mechanism of resistance to oxymino-cephalosporins and aztreonam in Gram-negative bacteria. The most important classes of ESBLs are TEM, SHV and CTX-M. In the early 2000-ties a significant increase in the prevalence of ESBL producing E. coli was observed particularly in urine samples, in paedatric units of the University hospital in Split, Croatia. This observation gave rise to an extensive study which was performed to characterize ESBLs in uropathogenic isolates of E. coli in the paediatric population of this hospital. That included the investigation of virulence factors and the analysis of the types of β-lactamases. During the two-year study period, 77 ESBL-producing E. coli strains were recovered from urine samples of febrile children with significant bacteriuria. The strains were found to express adhesins and to produce hemolysins in high percentage. Most of the strains were resistant to bactericidal activity of human serum which is important virulence factor. The most prevalent type of ESBL was SHV-5. Conclusions based on the results of the study was that hygienic regulation, reduction of antibiotic consumption and surveillance networks should be recommended in order to limit the spread of ESBL positive E. coli in the hospital departments involved.
