Impaired neuroimmunomodulation in nasal mucosa and nasal polvps.
Introduction : Topical application of capsaicine (extract of hot red chili pepper) reduces the size of nasal polyps and prevents after the surgery nasal polyps regrowth. Mechanism involved is due to depletion of sensory neurons and dorsal sensory root ganglia from their neuropeptide content and cauterization of posterior nasal nerves reduces statistically significant the endoscopic recurrence of nasal polyposis (p=0.016). We presumed that both mechanisms in reducing polyps and their recurrence is the same we tried to explain these events using the published experience and theories from other authors, known immunology events, our tots and c1inical experience. Material : Over 100 articles published in recent fifteen years regarding : Immunology, structure, cell and metabolic products of nasal polyp. Immunology, physiology, and pathophysiology of immunocompetent cells, nerve injuries and reparation. Rcsults : Cascade of immunological events which has proof in findings published by authors worldwide. Conclusion : Constitutional or acquired impair of production of different neurotransmitters in the nerve endings induces impaired immunological function of different immunocompetent cells resulting in polyp formation after inflamation with both bacteria and fungi and allergic sensitization. Remodeling and reinnervation can normalize the neurotransmitter content and restore normal immunological function. Experimental and clinical trials should be pointed and carried out to elucidate and confirm our theory.
