Hypertonic Buffered Saline Solution in the Diagnostic and Treatment of Rhinosinusogenic Headache.

INTRODUCTION: Isotonic and hypertonic saline is widely used in local medical treatment of different conditions of respiratory tract. We noticed that our patients having CT scan without criteria for chronic rhinosinusitis, had improvement of thir headache after treatment. For that reason we started with obligatory nasal irrigation in all our patients who had normal CT scan finding and headache as a symptom. Scope was to determinate weather the headache is rhinosinusogenic origin or not. In that way we tried to establish solid criteria for possible surgical treatment of rhinosinusogery caused headaches. MATERIAL: 93 patients with CT scan findings without criteria for chronic rhinosinusitis were treated for a period of three weeks with buffered hypertonic saline two times daily. Prior of CT scanning full patient examination and data were recorded on a examine from including: anamnesis data, symptoms and signs data, average headache frequency during the week in the past eight weeks, average need for analgesics during the week in the past eight weeks and average headache score indicated on visual analogue scale (VAS) graduated from 0 to 100 (0 for none and 100 for very strong pain) before the therapy and during the therapy. RESULTS: Average headache frequency during the week before therapy was 4, 06 and during therapy 1, 95. Average need for analgesics during the week before therapy was 2, 62 and during therapy 1, 00. Average headache score indicated on visual analogue scale (VAS) before therapy was 65, 8 and during therapy 36, 8. CONCLUSION: Our results are showing statistically significant decrease in headache frequency, need for analgesics and headache intensity score in our patients during the treatment with buffered hypertonic saline. When we have a patient with CT scan with no criteria for chronic sinusitis with signs and symptoms, treatment with buffered hypertonic saline is a treatment of choice to reduce patients headache and to distinguish weather or not is the headache of rhinosinusogenic origin.
