Endonasal surgery for sinonasal tumors.

Introduction: At our department we started with endonasal endoscopic sinus surgery in 1994. In past 10 years we used this technique for the treatment of sinonasal tumor as well. Material: 20 patients were operated for sinonasal tumors. 6 of them had malignant tumors and 14 had benign one. 12 were male and 8 were female. Mean age was 53, 4 years (21 to 81). Follow up was from 6 to 58 months, mean 25. In all cases only endonasal surgery was performed using endoscopes. Results: In the benign tumor group 7 patients had osteoma (4 ethmoidal and 2 frontal sinus), one cavernous haemangioma, inflamed fibroma and pyogenic granuloma. 5 patients had inverse papilloma. In the malignant tumor group was two carcinoma, one extramedullary plasmacytoma, two papillary adenocarcinoma and one adenocarcinoma. In the benign tumor group one patient with inverse papilloma had recidive and malignant alteration. In the malignant tumor group four patients died. Three due to heart failure (two astmatic) and one due to recidive. Two patients are disease free five years after treatment. Conclusion: Endonasal endoscopic or microscopic aided surgery for inverse papilloma in selected cases is world wide established method. Results are better than with open technique. In cases with malignant tumor this kind of surgery can offer in combination with other treatment modalities in strictly selected cases, radical, safe and less disabilitated surgery.
