Ultrasonography evaluation of hip joint articular effusion and articular sheat thickness in hemodialysis patients
Introduction Patients with chronic renal failure suffer from a number of complications mostly involving musculoskeletal system consequentially to renal osteodystrophy and hemodialysis associated amyloidosis. Aim The aim of the present study was to measure hip joint articular effusion and articular sheath thickness by ultrasonography in patients with chronic renal failure, and to assess the effect of hemodialysis duration, patient age and serum (32- microglobulin concentration on the thickness of these structures. Material and methods Thickness of the hip joint articular sheath and articular effusion were measured by ultrasonography in 106 patients on hemodialysis. Patients were divided into three groups according to the length of hemodialysis (<36, 36-72 and >72 months) and three groups according to age (18-50, 51-65 and >65 years). The measured values were compared with those recorded in 51 control subjects. Serum (32- microglobulin concentration was determined at the beginning of hemodialysis. Results Thickness of the hip joint articular sheath and articular effusion increased with the length of hemodialysis. There was no correlation between patient age and morphological changes in the hip region, or betvveen serum (32- microglobulin concentration and articular effusion in the hip region. A statistically significant correlation betvveen P2- microglobulin concentration and articular sheath thickness was found in the group of patients undergoing hemodialysis for up to 36 months, but not in the groups of patients on hemodialysis for 36-72 and >72 months. A longer duration of hemodialysis was associated with a higher serum (32-microglobulin concentration. Conclusion Articular changes in hemodialysis patients can be identified and followed-up by ultrasonographic determination of the hip joint articular effusion and articular sheath thickness. These alterations can be observed in the early stages, i.e. before clinical manifestations ofthe disease.
