Our experience in management of Fournier's gangrene.
This article presents two successfully managed cases of Fournier gangrene, in last three years, on Department for Surgery General Hospital "dr. Josip Benčević" in Slavonski Brod. Two male patient (age 47, 58) with anorectal etiology of Fournier's gangrene were diagnosed and treated on our department. Diabetes mellitus was present as a comorbid condition in one. After urgent surgical intervention, the treatment plan included aggressive resuscitation, repeated planed debridement (5.6) and broad spectum intravenous antibiotics. Diversion procedure (temporary colostomy) were performed in one due to extraperitoneal rectum lesion. Successful outcome in treatment of a patient with Fournier's gangrene can be achieved by combining aggressive surgical and medical management. Urgent surgical intervention with radical debridement, frequent repeated debridements associated with prompt fluid resuscitation and, rapid initiation of broadspectrum intravenous antibiotic therapy are milestones in treatment of this surgical emergency.
