Severity of Parkinson's disease as the risk factor for both cognitive and depressive disorders

Introduction: Besides motor symptoms, depression and cognitive disturbances often develop in patients with Parkinson's disease (PD). Due to different extent of the disease, standardised measures of the disease severity are developed. The objective of this study was to detect wheter the severity of PD contributes to cognitive decline and depression. Subjects and methods: Thirty patients with PD were evaluated. The severity of parkinsonian symptoms was assessed using the subscales of the Unified Parkinson Disease Rating Scale (UPDRS), Hoen and Yahr Scale and Schwab and England Activities of Daily Living Scale, while the level of depression was estimated using the Montgomery Aasberg Depression Rating Scale (MADRS). The P300 component of event-related potentials (ERP) has been analyzed in order to assess the cognitive function. Results: The level of depression as measured with MADRS has shown the high level of correlation with the Hoen and Yahr scores and UPDRS II subscale (Activities of Daily Living). Increased P300 latencies were seen in majority of patients, strongly correlated with the higher scores of MADRS. Motor symptoms did not prove to be significantly correlated with abnormal findings on ERP, while patients with higher score of the MADRS showed a strong tendency towards the prolonged P300 latencies. Conclusions: Given the significant correlation between depression and performance in daily functional activities, we believe that an evaluation of PD patients for depressive disorders becomes necessary for a better therapeutic outcome. Emotional disorders in PD seem to be associated with the decreased level of cognitive performance.
