The clinical use of fine needle aspiration cytology (FNA) for diagnosis of embrional tumors in children
This study presents the results of FNA oe imprint method in a series of 112 children with embrional tumor. We diagnosed 38% (43/112) neuroblastoma (NB), 5% (6/112) Ganglioneuroblastoma (GNB), 8% (9/112) Sacrococcygeal teratocarcinoma, 11% (13/112) PNET, 8% (7/112) Extra- osseus Ewings sarcoma (EES), 12% (14/112) Rhabdomyosarcoma (RMS) 4% (5/112) Hepatocarcinoma , 5% (6/112) Hepatoblastoma, 4% (5/112) Yolc sac timor, 2% (3/112) timor and 0.9% (1/112) thymoma. The diagnostic accuracy in our series was 98% in cases with adequate smears. In 46% (51/112) of cases we used aspiration cytology in preoperative diagnosis and in 54% (61/112) we used aspiration cytology or imprint method during operative procedure. In diagnosis of NB, GNB, RMS, EES, PNET, Hepatocarcinoma the cytological spectrum varied from undiferentiated small timor cells to mature ganglion cells to spectar of great cells with abundant cytoplasm and numerous mitosis. Imunocytochemistry confirmed the cytological diagnosis in 18 of (43) NB, 5 of (14) RMS, 5 of (7) EES, 8 of (11) Hepatic tumor and 7 of (13) PNET. Elevation of catecholamine metabolites in urine was detected in all children with a cytological diagnosis of NB, and a high level of AFP was detected in all children with a cytological diagnosis of Hepatocarcinoma. In our series of children with embrional tumours we found pathohistological confirmation in 95% of the tumors. Conclusion: aspiration cytology or imprint method in conjuction with imunocytochemistry offers a safe, rapid, simple diagnostic method in diagnosis of embrional tumors in children.
