Total and ionized magnesium concentration in serum and lymphocytes of alcoholic patients

Hypomagnesemia and cellular depletion of magnesium causes many symptoms in patients that suffer of chronical alcoholism. The aim of this study was to compare the concentration of total and ionised magnesium in serum and lymphocytes because the magnesium depletion in organism is not always seen in the serum. Total (Mg) and ionised (Mg++) magnesium were measured in serum and lymphocytes of healthy controls (n=60) and in the group of alcoholics (n=30) on first and last day of hopitalisation (7-14). Usual biochemical parameters (ALT, AST, ăGT) confirm us that patients were abstained during hospitalisation. Results showed that concentration of total (0.81ą0.02 mmol/L) and ionised (0.46ą0.01 mmol/L) magnesium in serum of chronic alcoholics statistically (p<0.01) decreased compared to helthy control values (0.87ą0.009 mmol/L ; 0.53ą0.005 mmol/L). In patients lymphocytes total (13.03ą1.5 mmol/g prot.) and ionised (0.96ą0.1 mmol/g prot.) magnesium statistically (p<0.01, p< 0.05) decreased toward controls (20.79ą1.1 ; 1.21ą0.08 mmol/g prot.). Decrease of magnesium concentration is marcedly expressed in lymphocytes (total 37.3%, ionised 20.6 %) than in serum (total 6.5 %, ionised 10.9 %). During the hospitalisation serum magnesium concentration did not showed significant changes, while in lymphocytes values still decreased (total 58.3 % ; ionised 29.8%) toward the first day of abstaination. We can conclude that determination of magnesium in lymphocytes is better for evaluation of magnesium concentration in organism of alchoholic patients. Measurement of ionised magnesium is useful for the monitoring of abstaination. During the period of 14 days of hospitalisation magnesium concentration still decreased but ratio Mg++/Mg showed that in lymphocytes ionised fraction rised during therapy.
