The Importance of Recognizing Paraneoplastic syndrome in Patients with Symptoms of Rheumatic Disease
Paraneoplastic syndrome (PNS) is a disease or symptom that is the consequence of the presence of cancer in the body, but is not due to the local presence of cancer cells. Neoplastic cells can produce a variety of peptides that exert biologic actions at local and distant sites and can elicit responses that cause a variety of hormonal, hematologic, dermatologic and neurologic symptoms. The importance of PNS must be recognized as it precedes the appearance of cancer in about 50% of cases, sometimes prompting the diagnosis. Paraneoplastic disorders can also add a significant degree of morbidity to the cancer patient and must be recognized by the physician to be included in the therapeutical treatment plan. We present a patient who first arrived with asymmetric migratory polyarthritis of the hands and symptoms of Sicca Syndrome. After further investigations he was diagnosed with a malignant disease.
