Objective structured clinical examination (OSCE) as a part of GP/FM specialist exam: Croatian experience
preparatory part, the trainees should in advance submit a portfolio, written essays, progress report and evaluation of the clinical competences signed by the trainers. The final exam consists of three parts: written test, practical and oral. 120 questions, EMQ and EMQ, are designed to asses the higher level of knowledge. Five minutes, OSCE stations, around 22, are designed to measure different psychomotor skills necessary for GP's work. The oral exam is taking place in front of three members jury of experienced GPs, mostly medical teachers. The aim of this study is to analyse the obtained OSCE scores during four years period (2005-2008). We reviewed the check-lists with scores for all 453 (21 OSCE exams) candidates during that period. 5 – 20% of the candidates did not obtain the passing scores. The averrage scores obtained at a specific OSCE station are: intra-articular injection – 70, 4% ; How to say No” – 77, 9%, ECG - 88, 2%, otoscopy – 95%, fundoscopy – 85% ; Family Apgar – 91, 0% ; CRP in babies – 96, 3% ; Heimlich – 96, 1%. We found statistical differences in the scores obtained for four kind of skills: physical examination (Mean 87.98, SD 11.8), interpretation of diagnostic tests (Mean 83.47, SD 16.6), therapeutically (Mean 88.82, SD 13.2) and the skills of the patient's education (Mean 86.15, SD 15.3). We also found higher scores in the skills prior practiced at Skills lab (Mean 89.33, SD 10, 15) than the skills practiced at clinical part of training (Mean 85, 79, SD 15, 35). There are a room for improvement of the OSCE part of the specialist exam: to better equip the Skill lab, to pay more attention to the skills training during the clinical parts of training and to introduce more complex skills (competence) into the exam.
