Spontaneous cytokine release from sinus and polyp samples of patients with chronic rhinosinusitis

Background: Chronic rhinosinusitis (CRS) is a multifaceted disease, where interaction of different predisposing factors and comorbid disorders may have impact on symptoms profile and objective findings, including type and severity of inflammation. The pattern of inflammation, orchestrated by Th1 and Th2 profile of cytokines, differs in two major phenotypes ; CRS with and without nasal polyps, but is also influenced by the treatment and by different predisposing factors, like asthma and allergy. The aim of this study was to compare inflammatory profile of different CRS phenotypes in vitro, on clinical samples obtained from CRS patients with and without nasal polyps, and to evaluate the impact of treatment with topical steroids. Study design: Sinus mucosa and nasal polyp tissue from 20 CRS patients was sampled during endoscopic sinus surgery. Samples (24 specimens of ethmoid sinus mucosa, 6 specimens of maxillary sinus mucosa, and 15 specimens of nasal polyps) were cut into small pieces and incubated in culture medium for 48 h. Spontaneous release of a panel of cytokines was measured in the supernatants. Patients were subgrouped according to presence of asthma, allergy, and topical steroid treatment prior to surgery. Those that stopped topical steroid 4 weeks before the surgery were considered not treated. Results: Comparing sinus and polyp samples from allergic and non-allergic patients, Il-a, Il-7, Il-15, interferon gamma, TNF alfa, Il-6 and Il-8 were significantly higher in non-allergic, and Il-4 and IL13 in the allergic. Comparing treated and non-treated polyps, Il-6, interferon gamma and TNF alfa were significantly higher in the treated samples. The release of Il-6, Il-8, TNF alfa and MIP1 was significantly higher in treated CRS samples. Conclusions: Multiplex cytokine analysis of sinus and polyp mucosa in vitro may demonstrate significant difference in the pattern of inflammation and impact of treatment in patients with CRS and nasal polyps.
