Perforin mediated cytotoxicity in the development of non ST-elevation myocardial infarction
This study was aimed to investigate the inflammatory status of patients with myocardial infarction (MI) with non-ST segment elevation (NSTEMI) during the first month after the coronary artery thrombosis by analyzing human serum C-reactive protein (hsCRP) and the frequency of inflammatory and cytotoxic molecule perforin within peripheral blood lymphocyte subpopulations and perforin mediated cytotoxicity. The expression of perforin in peripheral blood lymphocytes, (NK cell subsets, T and NKT cells) and perforin-mediated NK cell activity against NK sensitive cell line were measured by flow cytometry on the day 0, 7, 14, 21 and 28 after acute MI in group of patients with NSTEMI treated with anti-ischemic drug therapy. Concomitantly, pro-inflammatory status of the disease was followed by analyzing hsCRP using imunoturbidimetric method and Dimension Xpand analyzer. In the patients with NSTEMI gradual regression of P expression in lymphocytes was proved in both NK subsets (CD- CD56+dim and CD3-CD56+bright), T and NKT cells towards day 28 after myocardial infarction. The same fluctuation pattern was found in hsCRP. In patients with NSTEMI cytotoxicity was elevated the 21st day after the coronary thrombosis when compared to day 7 and 28 or to healthy examinees at effector: target ratio 50:1. The addition of anti-perforin mAb blocks almost completely NK cell killing except on day 21. Profound, one month long inflammatory reaction was found in NSTEMI patients in terms of the perforin expression and perforin mediated cytotoxicity which was in accordance with hsCRP fluctuations.
