"Foreign body" in the nose after cleft lip therapy
We present a case of an alginate foreign body in a previously unrecognized persistent oronasal fistula in a 20-year old patient who was previously treated for a cleft lip and palate. The patient presented himself to our department with a left side upper lip swelling and pain combined with strong fetor and puss secretion coming from his nasal cavity for the last six months. Previously the patient was treated by an otolaryngologist and an MSCT scan of the head and neck was performed which had shown a mass in the midline of the patients maxilla. Patient history has revealed that two years prior he underwent alginate impression taking for orthodontic treatment. After a thorough physical examination a fistula in the hard palate was identified and inside a blue mass. The foreign body was successfully surgically extracted and the fistula was closed under general anesthesia. The patient was discharged from hospital in a good state with no inflammatory signes. Great care must be taken in patients who were treated for cleft palate when planning a prosthetic or orthodontic therapy.
