Spontaneous orgasms in postmenopausal woman. A case report.

Abstract Body: In this report we present the case of a 49-year old patient who complained to her gynaecologist for having uncontrollable spontaneous orgasms during the previous eight months. The interesting point of this case is that these patients even though clearly suffering from primary neurological disorder, usually contact the gynaecologist first due to the nature of their symptoms. Furthermore, since cases like this are extremely rare, they represent a valuable contribution to clinical practice. ANAMNESIS: The patient has not been pregnant. Menopausal since three years ago. Treated for epilepsy in childhood. SYMPTOMS: The patient describes these attacks as a sensation of irritation in the area of vulva, followed by flushing and tachycardia. The attacks have no clearly determined trigger, and are not especially pleasurable for the patient since they are entirely beyond her control. The onset of the attacks is unpredictable and they sometimes come in series of up to three. GYNAECOLOGICAL EXAMINATION: Condyloma have been found in the perineal region and were treated with electrocoagulation. No other specific findings. Transvaginal colour doppler ultrasound examination was normal. Hormonal status in accordance with menopause. Estradiol cream was prescribed locally. WORK-UP AND TREATMENT: As orgasms continued to appear, the patient was referred to neurologist. Neurological status was normal. Electorencephalography showed an epileptic focus in the right frontotemporal area. Magnetic resonance (MR) was indicative of aneurysm in the siphon of left internal carotid artery which was confirmed by MR angiography. The patient is scheduled for endovascular surgery, and is on antiepileptic drug treatment.
