Speech disorders in patients with ischemic stroke

Background: To study the characteristics of speech disorders in a cohort of patients with a first-ever ischemic stroke. Methods: 178 patients (84 females and 94 males) admitted to the Osijek neurology department for a first-ever ischemic stroke in 2010 were included. Neurological examination of language disorders was performed using Boston test. Frequency of speech disorders in different subtypes of ischemic stroke was analyzed. Results: 113 (63.5%) of patients included in this study had speech disorders (females 66.7%, and males 60.6%). The more frequent clinical presentations were expressive-receptive aphasia (29.8%) and dysarthria (19.1%), whereas mainly expressive aphasia (12.4%) and mainly receptive aphasia (2.2%) were less frequently observed. According to a subtype of ischemic stroke (TOAST classification) speech disorders were most frequent in large vessel (77.6%) and cardioembolic (73.6%) strokes. Expressive-receptive aphasia was most frequent in large-vessel (51%) and cardioembolic (32.1%), mainly expressive aphasia in cardioembolic (20.8%) and dysarthria in small-vessel (26.2%) strokes. Conclusions: Speech disorders are very frequent in patients with a first-ever ischemic stroke. Location and type of ischemic stroke strongly influence speech disorder subtypes. Stroke aphasia is more likely associated with superficial middle cerebral artery stroke while dysarthria is associated with small-vessel stroke.
