Ileocecal inflammation and Mycobacterium tuberculosis in sputum of young adult : Diagnostic and therapeutic implications

Isolated tuberculosis of gastrointestinal tract is a very rare disease, most commonly localized in the ileo-cecal region (over 85% of the cases). The aim of this case report is to highlight the difficulties in the differential diagnosis of ileocecal tuberculosis and Crohn' disease. A 23-years old man, with history of weight loss, abdominal and low back pain, night sweat, fever, cough and mucous and bloody diarrhoea for some three months, was hospitalized. He had a positive family history (mother) of pulmonary tuberculosis. Laboratory findings were: elevated C reactive protein, raised sedimentation rate, a normocytic, sideropenic anaemia ; other standard biochemistry was normal, chest X-ray was also normal and the acid-fast bacilli test of sputum was negative. Microbiological examination of sputum was confirmed Mycobacterium tuberculosis and we started antituberculostatic treatment (rifampicin, ethambutol, pyrazinamid). Following the antituberculostatic treatment, the patient's abdominal and general condition improved with disappearance of all symptoms.
