Standard therapeutic procedures in pharmacotherapy of schizophrenia

Introduction: Despite modern approach in schizophrenia therapy assumes integration of psychopharmacotherapy, psychotherapy and psycho-social approaches, iz is wideely accepted that pharmacotherapy plays the most important role in this. Pharmacotherapy of schizophrenia is avaible for almost a half of the century, and over this period numerous products displaying clinical effectiveness have been synthetized. Over the last ten years so called novell antipsychotics were introduced in clinical practice. Increase in number of available treatements consequently multiplied the variety of approaches in shizophrenia therapy. Aiming to train professionals in optimal antipsychotics choice, a number of professional associations have elaborated standard procedures of pharmacotherapy in schizophrenia. That was recognised as possibility for improvement of professional practice in Croatia, and standard procedures in pharmacotherapy of schizophrenia, that are along with Croatian health system and insurance practices, were developed. Methods:In addition to expertise and experience of Croatian professionals in therapy of schizophrenia the folowing sources and methods have been used in developement of standard procedures: all available literature of most respectable professional associations ; analyses of published sources on efficacy, tolerability, safety and pharmacovigilance of antipsychotics ; epidemiological and pharmacoepidemiological available in Croatia and pharmacoeconomic data from local and foreign sources. The status of individual product in relation to registration process in Croatia and its status in reference to the positive list of the compulsory health insurance were also taken into account. The "Croatian standard procedures" of pharmacotherapy of schizophrenia were displayed in relation to the course of the disorder, patinet`s demographic characteristics, concomitant somatic illnesses, patinet`s compliance, therapy outcome and adverse events. The restrictive factors in elaboration of standard procedures were unavailability of conciderable number of products on local market, lack of existence of novell antipsychotics in depot form, and also the price of some medicines. Conclusion: The committee of experts appointed by the Croatian Association for Clinical Psychiatry at the Croatian Association of Physicians in cooperation with the Ministry of Health has developed standard therapeutic procedures in pharmacotherapy of schizophrenia. Elaborated standard procedures are along with those developed by the most respective international professional associations, but compliant to local proffesional practice, health system structure and available resources.
