Urgent transport of patients with STEMI for primary percutaneous coronary intervention- Croatian experiences.

INTRODUCTION: Transporting the patients suffering from acute STEMI up to 150 km distance away to the high volume centers, which can provide them on the 24/7 basis with prompt, high-quality primary PCI is safe and feasible and it proved to have achieved better results than the standard medicamentous treatment. The total population of Republic of Croatia is 4.437.460 inhabitants (year 2001), with total area of 87.609 sq. km, and gross domestic product per capita of 6.377 US $ (year 2003). Despite new network of freeways, there is a still a problem with geographically isolated regions, especially the mountains and islands, which may affect the availability of providing health care to the citizens especially in urgent situations. Since middle of the 2005, the Croatian Primary PCI Network (CPPN) was introduced into the system of the Health Institution of the Republic of Croatia using step-by-step and mutual communication principles. The main goal is equal quality treatment of acute STEMI in all parts of Croatia. MATERIALS AND METHODS: Between middle of the 2005 and the end of 2006. Four high volume PCI centers (three in capitol Zagreb, and one in city of Rijeka) or more than half Croatian citizens were included in CPPN. In that time patients with acute STEMI were transported in those centers from outside regions and were treated with primary PCI. CONCLUSIONS: The first step of the CPPN implementation ensured the best and equal treatment of acute STEMI for more than half of citizens of Croatia. Goal of the next step is extension of the CPPN to the entire region of Croatia.
