Biological and psychosocial status of patients with irritable bowel syndrome and inflammatory bowel diseases

The aims of this study were to determine the relationship between biological markers and disease severity as well as specific and general HRQoL for IBD and IBS patients and to determine the differences between IBS and IBD patients in biological status and HRQoL. 64 IBD and 86 IBS outpatients of the Clinical Hospital Center Rijeka, age range 19 to 75, participated in this study. Participants filled up the SF-36, IBDQ and IBS-36 as well as specific disease’s activity indexes. The laboratory tests for haemoglobin, serum iron, ESR, platelets, C-reactive protein and albumin were used. ESR and albumin were positively correlated with disease activity index and negatively with the mental component of the HRQoL for patients with CD ; the levels of serum iron in patients with UC were negatively correlated with the disease activity index and positively with the mental component of the HRQoL, while the levels of C-reactive protein were negatively correlated with the mental component of the HRQoL ; for IBS patients ESR was positively correlated with the IBS-36. ANOVA showed that IBS patients and patients with a lower severity index had a better physical HRQoL than IBD patients and patients with a higher severity index. We conclude that the more prominent sign of inflammation is related not only to higher symptom expression but also to the lower mental component of the HRQoL. Suprisingly, obtained significant correlation between ESR and disease-specific HRQoL in IBS patients point to the possible role of mild inflammation in symptom generation.
