The role of CD23 antigen in IgE regulation in atopic dermatitis patients

Atopic dermatitis (AD) shows a variety of humoral and cell-mediated immune dysfunctions, including elevation of serum IgE level; multiple positive immediate skin tests to a variety of antigens; CD8+T cells that disturb the regulatory function of CD4+T cells concerned with enhancement of IgE production; CD23 and CD21 antigen that participate in the control of IgE synthesis. Signas mediated through CD21/CD23 interaction seems to be very important in IgE antibody production. Aim was in 33 AD patients to investigate expression of CD23 and CD21 molecules on B cells, possible correlation between total IgE and CD23 antigen and existence of T cell activation (HLA-DR expression). Three colour immunofluorescence analysis on a flow cytometer using "Whole blood" immunoluorescence staining with monoclonal antibodies for surface marker analysis and total serum IgE with Prist kit were used. Patients with AD had increased % of total CD23 lymphocytes and % of CD23+ in B lymphocytes when compared with controls, CD23+lymphocytes positively correlated with total IgE levels in sera od AD patients. Increase percentage of CD23+in B lymphocytes in patients with normal IgE levels suggests the existence of B cell activation that is indipendent od IL-4. Experimental evidence that factors important for IgE regultation change the phenotype of B cells defined by expression of CD21 and CD23 antigen.
