DETECTING ENDOMETRIAL DISORDERS BY UTEROBRUSH DEVICE

Endometrial carcinoma is the most common invasive neoplasm of the female genital tract, with a rising incidence over the past few decades. Pap test is not considered as a reliable detection method for those lesions. The aim of the study was to evaluate the efficacy of endometrial cytology using Uterobrush in the collection of the samples for the diagnosis of endometrial lesions. During 12 months period direct endometrial samples were collected from 162 perimenopausal or postmenopausal symptomatic patients at Gynaecology Department in our hospital. Results were compared with histology of the endometrium obtained with dilatation and curettage (n= 36) and hysterectomy (n=16) or the patients´ follow –up mainly by ultrasonography (n=97). All of the endometrial samples were adequate for cytological analysis, but histological diagnosis has not been established in 13 cases due to absence of endometrial tissue and they were excluded from the study. By Uterobrush method benign condition of the endometrium was recognized and confirmed in majority of patients (138/149 ; 92, 6%). Seven cancers (7/149 ; 4, 7%) were diagosed and confirmed by histology and no cytological false positive results were found. In our study the 4 cases of inactive endometrial polyps (2, 7%) were interpreted as hyperplasia without atypia. After using uterobrush device, patients who were followed by ultrasonography show no endometrial disorders or gynaecological symptoms. Endometrial cytology by using Uterobrush method can provide direct, adequate and representative endometrial sample. It can be used for evaluating patients with postmenopausal bleeding, as a screening method for patients at risk for endometrial or extrauterine cancer ; or for follow up of perimenopausal patients with an increase of the endometrium thickness detected by ultrasound. In that way cytology evaluate the endometrial condition and possibility of lesion progression and provide an indication for further examination.
