Considering peptidergic drugs, many cytokines, enzymes, clotting factors registered for the use as therapeutic agents in specific conditions, they are mainly given by injection, only occasionally by other routes. Also, to ascertain delivery and activity, they use different carriers, peptide+carrier complex may provide significant methodology/activity dilemmas. In this, since the first report (Sikiric et al, J Physiol Paris, 1993), we focused on stable gastric pentadecapeptide BPC 157. Since given alone in different species, without carrier, and effective when given parenterally, perorally and locally (i.e., cream), with a special structure (GEPPPGKPADDAGLV, MW 1419), it may be particularly important as a small anti-ulcer peptide in the whole GI tract (non degraded in human gastric juice more than 24h), liver and pancreas lesion, efficient in inflammatory bowel disease trials (PL 14736) and various wound treatment, no toxicity reported. In rats with esophagitis and failed function of both lower esophageal sphincter (LES) and pyloric sphincter (PS), BPC 157 increased pressure in both sphincter till normal values and reduced esophagitis. However, in healthy rats, it may decrease (PS) or increase (LES) pressure in sphincter. Interestingly, it has strong angiogenic potential, it may act protectively on endothelium, affect many central disturbances (i.e., dopamine and 5-HT system), the NO-system (either L-arginine and L-NAME effects), endothelin, act as free radical scavenger and exhibit neuroprotective properties. Also interestingly, its wound healing potential (expression of the early growth response 1 (egr-1) gene and its repressor nerve growth factor 1- A binding protein-2 (nab2)) could be seen in different tissue healing (i.e., GI-tract, skin, nerve, cornea, muscle, tendon, ligament, bone). Of particular importance may be different anastomosis and fistulas (i.e., gastrocutaneous, duodenocutaneous, colocutaneous) healing, and recovery of short bowel syndrome. As a likely consequence of its close interaction with NO-system (as endogenous cardioprotectant), it reversed various arrhythmias (i.e., digitalis) and heart failure (induced by doxorubicine). while in condition of either hypertension (L-NAME) or hypotension (L-arginine) BPC 157 application resulted with blood pressure normalization.
