The evaluation of skin prick test criterion for sensitization to dust mites
An usual criterion for positive skin prick test (SPT) reaction is a mean urtica diameter ł 3mm. We evaluated the accuracy of this SPT criterion regarding specific IgE values >0, 35 kUA/L (+sIgE). SIgE (UNI CAP 100, Pharmacia AB Diagnostics, Sweden) and standard SPT to Dermatophagoides pteronnyssinus (DP), Dermatophagoides farinae (DF), Lepidoglyphus destructor (LD) and Tyrophagus putrescentiae (TP). /Epipharm-ALK, Denmark) were performed in a random sample of 132 subjects, 105 men (mean age 39.4ą8.9 years) and 27 women (mean age 40.1ą8.5 years). Statistical analysis was done using correlation test and discriminant analysis. If mean urtica diameter ł 3mm was considered positive SPT (+SPT), correlation between +SPT and +sIgE was 0.58 for DP, 0.55 for LD, 0.43 for DF and 0.38 for TP (all p<0.001). Regarding +sIgE, this SPT criterion has an accuracy of 81.1% for DP, 78.8% for LD, 73.5% for DF and 75.8% for TP. SPT criterion ł 3.5 mm for DF discriminates correctly 80% of subjects, and SPT criterion ł 5 mm for TP discriminates correctly 81.8% of subjects. Our results suggest that SPT criterion ł 3mm is related to presence of false positive SPT reaction to DF and TP. Reevaluation of DF and TP allergen standardization or changing of SPT criterion for sensitization to DF and TP should be considered.
