The role of mucosal immunity in patients with oral lichen planus
OBJECTIVE: Oral lihen planus (OLP) is common chronic inflamatory disease involving T-cells mediated immunity, where mucosal immunity have specific role. The hypotheses was that evidence of changes in levels serum imminoglobulins may have reflect on local immunity, especialy on salivary imminoglobulins. The aim of this study was to deternm levels of salivary immunoglobulins IgA and IgG subclasses as indicator of mucosal immunity of patients with OLP. PATIENTS AND METHODS: This study was conducted on 30 patients (age 60.17 +/- 11.75) with clinically and histopathologically confirmed diagnosis of OLP and 30 healthy controls (age 56.16 +/- 11.82). Determination of salivary IgA1 and IgA2 was performed by radial immunodiffusion (RID), and salivary IgG1, IgG2, IgG3 and IgG4 by enzyme-linked immunosorbent assay (ELISA). Statistical analysis was done using Mann-Whitney U test and significance was determined as p values lower than 0.05. RESULTS: Salivary immunoglobulin IgA subclasss IgA1 and IgA2 in patients with OLP were significantly increased in comparison to the healthy control group Salivary immunoglobulins IgG subclasses IgG1, IgG2, IgG3 and IgG4 in patients with OLP were significantly increased with comparison with healthy control group. CONCLUSION: We might conclude that elevated levels of salivary immunoglobulins IgA and IgG subclasses IgA1, IgA2, IgG1, IgG2, IgG3 and IgG4 show that humoral immunity can be implicated in the pathogenesis of OLP by modulation of mucosal immunity in this oral autoimmune disease.
