Personalized pharmacotherapy in psychiatry

The hypothesis of every individual being special and different leading to heterogenity of diseases sets grounds for concept of personalized medicine. Personalized psychiatry follows the principles of personalized medicine. Constituent part of individualy adapted approach towards psychiatric patient presents itself thorugh personalized psychiatry. Development of pharmacogenomics and pharmacogenetics as well as nanotechnology based of them ensured implementation of presonalized medicine principles in psychiatry in greater extent than other medical disciplines. In field of pharmacogenomics, the greatest advancement was reached in research of genetic variability in drug metabolism. All predispositions are made for implementation of pharmacogenetic tests in routine practice. Pharmacogenetic testing for medications metabolizing thorugh two polymorph cytochormes P 450: CYP2D6 and CYP2C19 is of special significance due to their involvement for most adverse and ultrafast metabolism of psychopharmacs. The potential application of personalized medicine in psychiatry, supported by pharmacogenetics and pharmacogenomics, are: personalized medication choice, personalized dosage, individual anticipation of possible side-effects and personalized exstensive treatment with rehabilitation. The authors conclude how development of pharmacogenomics and pharmacogenetics as well as nanotechnology based od them, presents great step in creating presonalized therapeutic approach in psychiatry, although burden of applying most appropriate therapeutic agent and dosage remains on clinician decision. Pharmacogenetics can only help on making therapeutic decisions with one less unknown element.
