Therapeutic hypothermia (TH) has been used in various indicatons for decades and has a potential to improve supportive treatment of patients with the severe inflammatory diseases of the CNS. Potenital benefitial effects of hypothermia in these patients ensue from reduction of cerebral metabolic demands, reduction of inflammation and apoptosis of neurons, stabilisation of blood brain barrier, decrease of vascular permeability, reduction in production of free radicals and stabilisation of cell membrane. The ultimate consequneces of these effects are reduction of brain edema and intracranial pressure. Indication for TH in our patients was low breath holding index (BHI) measured by transcranial doppler (TCD) or Glasgow coma score ≤ 8 if the acustic window was absent. The effects of TH were monitored by daily measurment of jugular bulb oximetry and derived parameters, optic nerve sheath diameter and mean velocities of blood in cerebral vessels with calculation of BHI. Our preliminary results support the benefitial effect of TH on patients outcome.
