Cholesterol emboli syndrome : two cases report

Cholesterol emboli syndrome occurs in patients with generalised multiple atheromas, arising as a consequence of arteriosclerosis. It is more common in males than in females. The incidence increases with age, being rare before the age of 50. Risk factors for cholesterol embolism include hypertension, diabetes, aortic aneurysm and invasive vascular procedures. Cholesterol atheromatous embolism is a systemic disease resulting from cholesterol crystal embolization to many organs, including kidney. In addition to kidneys, atheroembolization may involve the skin, gastrointestinal system, and central nervous system. The outcome of renal disorder may be variable ; some patients deteriorate or remain on dialysis, some improve, and some remain with chronic renal impairment . We present two cases of cholesterol embolism after coronary angiography. Both patients clinically presented with acute renal failure with elevated blood pressure, creatinine and sedimentation rate. One patient had increased eosinophil count. Renal biopsies were performed which revealed characteristic needle-shaped cholesterol crystals in arterial lumina, surrounded by inflammatory cells. Corticosteroid therapy in both cases and haemodialysis in one case led to stabilisation of the renal function.
