CD38 level correlates with activation status in interstitial lung diseases (ILD)

The functional status of the lower airways constituents in ILD us incompletely understoood. The purpose of this study was to assess whether the selective elevation of CD38 on bronchoalveolar fluid (BALF) lymphocytes, that we previously reported, reflect ILD activity. By triple colour simultaneous staining with CD38 and HLA-DR mAb we analysed distinct BALF subset profiles from patients with active sarcoidosis (AS=16), inactive sarcoidosis (IS=15) and idiopathic pulmonary fibrosis (IPF=8) on highest CD4 and CD8 gated cluster and compared the dana with the same parameters in controls (C=7). Both CD4 and CD8 BALF cells bearing CD38 molecule were low in controls, IS and IPF, but in AS there was a dramatic and progressive increase of CD38. As the CD38 molecule may promote cell recruitment and drive, the local differentiation and proliferation of effector cells, we conclude that raised CD38 may be, at least in part, responsible for disease severity, because in sarcoidosis both lymphocytes and macrophages are involved in the local immunological processes. Moreover, the present dana suggest that double activated CD4+CD38+HLA-DR+ subpopulation play a relevant role in the onset of alveolitis and could serve as an additional indicator of disease activity.
