Parasthesia of the inferior alveolar nerve as a result of endodontic treatment of tooth 37
Parasthesia of N. Alveolaris Inferior due to Endodontic Treatment of tooth 37 This clinical case is about parasthesia of n. alveolaris inferior due to endodontic treatment of tooth 37. Twentysix year old female, specializing in radiology, came to Department of Endodontics and Restaurative Dentistry with severe pain in the left distal part of the lawer jaw. Clinical observation revealed open endodontic spaces of tooth 37 (with canals enlarged to file # 35), and tooth 36 with obliterated root canals. There was no swelling, no pain on percussion of tooth 36, and slight pain on percussion of tooth 37, with symptoms of parasthesia. Submandibular lymph node was slightly palpable. Patient has presented orthopantomogram of finished root canal filling of 37 performed three days ago and intraoral radiograph after dental emergency retreatment performed a day ago. Radiographs showed distal root of 37 closely positioned to mandibular canal and no strange particles in the periapical region of 37. The tooth 37 was irrigated with sterile saline solution and calcium hydroxide dressing was introduced into canals. The tooth 36 was instrumentated but mesial canals were still obliterated. Calcium hydroxide was aplicated into canals of 36. Teeth have been closed with temporary cement. On the second appointment patient reported increasing irradiating pain. Symptoms of parestesia became stronger. Vitamine B and corticosteroids were prescribed. There were no signs of swelling, only slight pain 37 on percussion. On her own initiative, the patient has taken computer tomography of this mandible area which revealed descrete rarefication of the bone closely to the apex of distal root of 37 with position of the apex immediately to mandibular canal. The patient has been monitored for two weeks in which period parasthesia subsided. The root canal therapy of 37 was finished using sterile saline solution and cold lateral condensation filling technique
