Orofacial pain caused by osteoarthirtis of temporomandibular joint and trigeminal neuralgia
This case report described diagnostics and treatment modalities in a patient with one and a half year follow up. The 78-year-old female patient came to the dentist in 2007 complaining of her painful TMJs bilaterally and crepitus in right TMJ only. The pain was occasional and caused limited mouth opening ; sometimes it prevented the patient from eating or brushing teeth. She attributed the pain to a new lateral ceramic bridge in the maxilla. By magnetic resonance imaging and computerized tomography, disc displacement without reduction in the left TMJ, and osteoarthritic changes in both TMJs were found. The stabilization splint, according to some, was indicated for the initial treatment of joint pain. After a neurological examination, she was diagnosed with TN. As a first line treatment, analgesia was achieved with carbamazepine as well as a significant improvement of the clinical picture.
